Office Giveaway & Promotional Guidelines
In-Office
To qualify as a winner for any of our office giveaways or promotions, patients must meet the following eligibility criteria:
- Be an Active Patient
- Be Up-to-Date on Preventive Care (including routine cleanings)
- Maintain Overall Good Financial Standing
- Maintain Overall Good Standing as a Patient
- Have a Zero Balance, or an active financial arrangement with a minimum monthly payment of $50
- Sign a Photo/Video Consent Form and agree to having pictures or videos taken, if applicable
Social Media
To qualify as a winner for any of our social media giveaways or promotions, patients must meet the following eligibility criteria:
- Maintain Overall Good Financial Standing, if applicable
- Sign a Photo/Video Consent Form and agree to having pictures or videos taken, if applicable
- Be Able to Pick Up the Prize in Person at our office
